PAMCMF favors charitable request applications from organizations and individuals that serve to benefit the geographic areas served by PAM, and that:

  • Are directly related to health, wellness and health education. 

  • Are directly related to access to clinical care and clinical technologies.

  • Assist in enhancing health care services in rural and underserved areas.

  • Provide support and strengthen the delivery of health care services.

  • Produce high quality, cost-effective, measurable results

  • Enrich to communities served through educational grants and scholarships for various health professions

For calendar year 2024, PAMCMF will accept charitable request applications (and supporting documentation) From January 1, 2024 – September 30, 2024.

  1. Review the PAMCMF Charitable Giving Criteria (see above). Applicants are required to review the full criteria prior to submission of their Application, and to confirm the understanding of the terms, conditions and Applicant’s obligations found therein.

  2. Download, fill out, and sign the 2024 PAMCMF Charitable Request Application (see below).

  3. Collect all required documentation, including signed application, and any other supporting documents mentioned in the Application.

  4. Email info@pamcmf.org with your pdf of the Application and required documents in .pdf, .jpg or .doc formats.
