PAMCMF favors charitable request applications from organizations and individuals that serve to benefit the geographic areas served by PAM, and that:

  • Are directly related to health, wellness and health education. 

  • Are directly related to access to clinical care and clinical technologies.

  • Assist in enhancing health care services in rural and underserved areas.

  • Provide support and strengthen the delivery of health care services.

  • Produce high quality, cost-effective, measurable results

  • Enrich to communities served through educational grants and scholarships for various health professions

INSTRUCTIONS

  1. Review the PAMCMF Charitable Giving Criteria (see above). Applicants are required to review the full criteria prior to submission of their Application, and to confirm the understanding of the terms, conditions and Applicant’s obligations found therein.

  2. Download, fill out, and sign the appropriate application below for the type of award you are applying for: grant award or educational award.

  3. Collect all required documentation, including signed application, and any other supporting documents mentioned in the Application.

  4. Email info@pamcmf.org with your pdf of the application and required documents in .pdf, .jpg or .doc formats.

Grant AWARD Requests

For calendar year 2025, Physician Associates of MidMichigan Community Medical Foundation (PAMCMF) will accept grant award applications (and supporting documentation) in two cycles, from January 1 – May 31, 2025 and from June 1–October 31, 2025. Applications submitted in full by May 31st will be reviewed and approved by the PAMCMF Board in June, after which time all applicants will be notified. Applications submitted in full by October 31st will be reviewed and approved by the PAMCMF Board in December, after which time all applicants will be notified.

Educational Award Requests

For calendar year 2025, Physician Associates of MidMichigan Community Medical Foundation (PAMCMF) will accept all educational award applications (and supporting documentation) January 1 – May 31, 2025. Applications will be reviewed and approved by the PAMCMF Board in June 2025, after which time all applicants will be notified.
